Delivery Issues Form

Use the form below to report the following:

  • Missed Stop
  • Incomplete Pick-Up (not all outgoing items picked up)
  • Missing Items
  • Damaged Items
  • Received Wrong Items
  • Excess Bins
  • Additional Bins Needed
  • Library Closure
  • Temporarily Suspend Delivery

In the case of a library closure, libraries should notify OLIS as soon as possible, but no less than 24 hours before a scheduled delivery.

You can expect to hear back from one of our team members within one to two business days.
